Electronic Waveform Lab understands the importance of protecting your personal information. This notice relates to an incident that may have involved some of that information, including measures we have taken and some steps that you can take in response.

What Happened?

On April 11, 2020, we discovered a ransomware incident that affected some of our computer systems. We immediately took steps to secure our network, began an investigation, and hired an experienced computer forensic firm to assist.

What Information Was Involved?

The unauthorized actors may have obtained some client information, including your name, address, date of birth, insurance information, and in some cases, limited diagnosis or treatment information. Electronic Waveform Lab’s servers have been restored, and no data has been lost due to this incident.  

What We Are Doing

We continually evaluate and modify our practices to enhance the security and privacy of your personal information. To help prevent something like this from happening in the future, we are instituting additional security measures.

What You Can Do

Although we do not believe your information has been or will be used, as a precaution, we recommend that you review any statements that you receive from your healthcare providers. If you see services that you did not receive, please contact the provider immediately.
